As a nominee in the 2022 Community and Human Rights (CAHR) Awards, you are requested to provide documentation to buttress your nomination.

Please note the guidelines below:

• All materials must be added to a folder and uploaded as a zipped file
• The maximum filesize for the uploaded document is 3MB
• Reference materials can either be a photo, a short video, or a text file in either .pdf, .png, .jpg, .mp4, .docx or doc format
• Reference materials should not be password-protected
• All text files should be no more than 3 pages with accurate description of your activity
• Supporting images should be no more than 5 and uploaded as a zipped file. It is advised that you take time to select the images that better buttress your work
• Applicants who submit forged or misrepresented documents will be immediately disqualified
